Profhilo is a hyaluronic acid (HA) based injection manufactured by IBSA, Switzerland. Although it contains the same hyaluronic acid ingredient just like Restylane skin boosters and many other dermal fillers, it is considered neither a skin booster nor filler.
Profhilo is a new hyaluronic acid product which is manufactured with a unique process which uses thermal crosslinking of low and high molecular weight hyaluronic acid. It is different from other skin boosters and dermal fillers due to its mode of action – it stimulates bio-remodelling of the collagen and elastin structures in the skin.
Hyaluronic acid is a natural polysaccharide in our skin. It is contained within the collagen and elastin structures of our skin and mainly acts as a reservoir to contain and hold onto water molecules and maintain tissue hydration. With ageing, the natural production of collagen and elastin slows down causing a decrease in the amount of hyaluronic acid in our skin. This then leads to the skin losing its elasticity, and starts sagging, creating a wrinkly appearance.
Profhilo contains the highest concentration of HA in a product in the market (32mg/mL). It is also a combination of both low and high molecular weight HA and is non cross-linked (unlike fillers). Through a proprietary process (NAHYCO® Hybrid Technology), the L-HA and H-HA chains in Profhilo interact with each other to form a Hybrid Cooperative Complex (HCC) with unique rheological properties. With this, a hyaluronic acid molecule with very unique properties are produced:
1. High HA concentration: Profhilo contains 32mg/mL of LmwHA and 32mg/mL of HmwHA – totalling 64mg in 2mls treatment
2. Extensive spreadability and low viscosity – well distributed all over the face once injected
3. Non cross-linked
4. Low inflammatory response and integrates well into our skin
5. Lasts about 30-45 days in the skin which equates to a longevity much longer than skin boosters.
Profhilo works by remodelling the skin and improving the quality of the skin itself.
Profhilo achieves bio-remodelling of the skin in 3 main ways:
– The high concentration of HA in Profhilo stimulates fibroblast cells to increase natural collagen and elastin production in the dermis
– The HA in Profhilo directly provides structural support to the skin via volumetric effect to reduce sagging
– The HA content in Profhilo acts as a reservoir for moisture and thus maintaining elasticity of the skin.
This is achieved by the unique manufacturing process in which a unique hyaluronic acid molecule called the Hybrid Cooperative Complex (HCC) is produced.
Skinboosters use the principle of replacing the hyaluronic acid we lose with ageing in the skin to restore hydration and elasticity.
Fillers on the other hand are designed to replace lost volume or fill up folds. Most fillers are also cross-linked to make them last longer.
Profhilo as a hyaluronic acid treatment also hydrates the skin like skinbooster treatments. However, the HCC go further by stimulating collagen and elastin in the skin which gives its skin tightening and rejuvenating effects. It works in the epidermis, dermis and superficial fat compartments to restore aged tissue without volumisation. It is a regenerative anti-ageing treatment which reverses ageing through bioremodelling. It is also non-inflammatory unlike biostimulators currently on the market such as Ellanse and Sculptra.
Once injected, Profhilo almost immediately gives improvements in skin hydration and fine lines.
Progressively, skin tightening and improvements in elasticity are also seen. Improvements in wrinkles and skin texture are typically seen after 8 weeks. Patients expect to see a dewy, glossy and plumper-looking complexion when the full effects show, which can take up to 12 weeks.
However, what Profhilo does not do is lift very saggy skin. It is not and should not be treated as a substitute for procedures such as threadlift/ fillers/ surgical facelift.
Profhilo can be used to treat both face and body. It is particularly indicated for treatment of the cheeks, jowls and necks where injection protocols have been studied and showed to give very good results.

Profhilo comes in individual pre-filled 2ml syringes. From the prior extensive studies done on the product, a special Bio Aesthetic Points (BAP) technique had been introduced to administer the injection.
What is exciting (for both doctors and patients) is that Profhilo only requires 5 injection points on each side of the face and neck to be administered. The technique chooses 5 anatomically receptive areas of the face with an absence of large vessels and nerve branches, minimizing risks whilst maximizing diffusion of the product into areas such as the cheeks, jowls and lower face. This represents an advantage over conventional skinbooster injection and mesotherapy techniques giving more patient comfort and less downtime.
After the doctor administers the injection, side effects are generally very mild (eg. Swelling, redness and occasional bruising). Raised bumps very quickly fade away over a few hours as the product diffuses itself all over the skin.
Minor pain and discomfort during injection is expected. This should be minimal as the needles used are relatively thin (29g).
There is usually no downtime after treatment with Profhilo and patients can typically resume their daily activities after the treatment, although there is a small risk of bruising.
Mild redness and soreness after the treatment is expected and will usually resolve on their own in a few hours after treatment. Bruising if it happens generally takes about a week to go away.
The effects of Profhilo usually last 6 to 12 months. It is best to repeat the treatment twice a year for maintenance.
Profhilo is suitable for anyone who is suitable for skin boosters or dermal fillers with mild to moderate skin ageing who wishes to improve skin texture, hydration and tightness. It works best when signs of ageing are just beginning to show. It is also safer than most HA-based treatments for skin ageing.
Men are also suitable for the treatment as it works well on thicker skin and does not feminize masculine features. However, due to the unique nature of skin conditions, it is best to consult your doctor before commencing treatment.
Yes, Profhilo can be done in combination with other treatments and in fact may complement energy-based devices such as Ultherapy and Picolaser as part of an overall rejuvenation programme. Do inform your doctor if you are undergoing other treatments or using other products so that your doctor is able to plan your treatment accordingly.
